Inflammatory Spinal Pain

Abstract
To present the characteristics of inflammatory spinal pain. Review of the scientific literatures that described the characteristics of inflammatory spinal pain and the classification criteria of spondyloarthopathy (SpA). Inflammatory spinal or back pain (ISP or IBP) is a characteristic of inflammatory spinal disorders. It differs from mechanical spinal pain in type of onset, onset age, choronicity, aggravating and relieving factors and appears in many skeletal disorders, not only inflammatory spinal disorders but also some mechanical spinal disorders. Although ISP or IBP is one of clinical manifestations in degenerative spondylosis, non-infectious spondylodiscitis, Bechet¡¯s disease, and SAPHO syndrome, the most common disease in which ISP or IBP manifests is SpA. Because all kind of clinical and laboratory features of SpA does not manifest simultaneously, high index of suspicion of SpA is necessary when the patient present with ISP or IBP. Various classification criteria of SpA in which ISP or IBP is included as one of major requirements are proposed. Inflammatory spinal or back pain that is insidious onset, in a patient younger than 40 years, persisting at least three months, associated with morning stiffness and improving with exercise is characteristic of inflammatory spinal disease.
